The Hawaii Family Court posted its Hawaii Civil Union Divorce forms in February 2012 (civil unions became available in Hawaii on January 1, 2012). Linked here, for Oahu and Maui (the Big Island and Kauai do not have any posted, but it should be noted that they have much fewer forms posted than Oahu and Maui). The civil union divorce actions (using the Court’s acronym, “CUDA”) will utilize forms that are nearly identical to the Court’s divorce forms, being primarily distinguished by the notation “civil union” in front on the word divorce, in most cases.
